The SF Palliative Care Work Group is celebrating National Health Care Decisions Day by inviting you to join us at an Advance Care Planning workshop.
The SF Palliative Care Work Group is a consortium of organizations that collectively encompass the regions most prominent health systems, community organizations, foundations and health plans. Together, its members aim to ensure that people facing serious illness have access to high-quality care consistent with their wishes & values and provides the tools and information needed to achieve this though community engagement and education.
